New Patient Registration
Our Practice List is open to everyone who is eligible to receive NHS treatment, however, if you wish to register from outside the boundary area of postcode SS8 then there are some important things you need to know:
- If we feel that it is not clinically appropriate or practical for you to be registered so far away from home then we may refuse registration.
- The practice will be unable to provide you with a home visit when you are not well enough to go to the practice yourself.
- You may, if your circumstances change and it is no longer clinically appropriate or practical for you to be registered away from home, be asked to consider registering with a practice closer to home where your care may be better provided at that time.
- Should you become too ill to attend in person or we are unable to assist you over the phone, contact NHS 111 who will direct you to the right service for your needs.
All applicants who are eligible to receive NHS treatment may register by completing the appropriate registration form available at the reception desk. To facilitate your registration please bring your NHS medical card if available with you. In addition please understand that we are entitled to refuse non-urgent/non-emergency treatment pending your registration. Emergency treatment will be provided irrespective of registration. Patients who are not eligible for or entitled to free NHS treatment will be offered our services at an appropriate fee. Relevant details are available from the reception desk.

Preliminary visit
As a registered patient you will be offered a preliminary appointment designed to introduce you to our services and to discuss, if you feel it is appropriate, any medical worries or problems you may have. At this visit you will be seen by a clinical member of our team whose function is to collect data about your health and to introduce you to the various facilities available to you. We hope to see all newly registered patients within three months of enrolment. May we request that you complete the brief preliminary visit questionnaire available from reception and bring it with you at the time of your appointment. Please bring a specimen of your own urine when you arrive for new patient screening appointment.
The Practice runs an appointment system with doctors, nurses and healthcare assistants. An appointment can be made in person or by telephoning the Surgery during opening hours. When you contact the surgery please ask the receptionist for their name in case you need to contact the surgery further.
As per N.H.S. registration requirements you will be registered with the Practice. However, you do have a right to express a preference to see any doctor.
